Year 9 Consent for a School Visit


As part of the integration of your child into the Senior School, we are proposing to take the whole of Year 9 out of school for the day. Pupils will be involved in a broad range of cross-curricula activities ranging from a soil analysis for geography coursework; trigonometric exercise for maths; art; film making; orienteering and a variety of scientific activities. Pupils will be made aware of safety hazards in the park, especially the deep tout lake. There will be no additional costs associating with this trip.

From: 08.45 to: 17.00 on Monday 17th September 2018

• I agree to my child taking part in this visit. I have read the information above and I agree to his/her participation in the activities described. I acknowledge the need for him/her to have responsibility.

• I understand that the staff in charge of this visit will refer to the medical information on the Pupil Health Form which I have supplied to the College Medical Centre.

• I will inform the group leader and the Medical Centre of any relevant changes in medical or other circumstances occurring between now and the group's departure.

• I consent to appropriately trained staff administering first aid, approved over-the-counter remedies and prescribed medication authorised by the Medical Centre, where appropriate.

• I consent to emergency medical or dental treatment if necessary.

• I understand the extent and limitations of the insurance cover provided.

Please provide us with the details of the most appropriate person we should contact, if we need to, whilst this trip is taking place.
